Welcome to our webpage to complete forms for your child or yourself. Please download the forms your doctor has requested for you to complete. Here are the steps. Kindly download the forms on your desktop. These forms do not require printing or scanning kindly Open “The Markup Toolbar” located on the right upper corner of the form to be completed (This way is faster allows you to add text directly on the forms) when you are finished please close “The Markup Toolbar” then save it on your desktop on “pdf” and upload the forms to Ava connect.

Sending a PDF document to your doctor through Ava Connect

You can attach a PDF document in "Messages" and send it to your doctor, in Ava Connect Portal. One important tip please request access to upload this file to your “ava connect”.

The Ages & Stages Questionnaires is a developmental screening tool that pinpoints developmental progress in children between the ages of one month to 5 ½ years.
